New Patients
Are there forms to complete for my first appointment?
For new patient visits, expect to show up 10-15 minutes prior to your appointment to fill out some paperwork and for our staff to log your insurance information. This paperwork can be filled out ahead of time if you wish by downloading the following forms:
- Patient Registration
- Payment and Cancellation Policy
- Notice of Privacy Practice
- Email Authorization Form
- Patient Agreement and Individual Treatment Plan
These can be faxed back to our office or emailed with “New Patient Registration” in the subject line.

What do I need to bring with me to my first appointment?
Please remember to bring the following items:
- Insurance Card
- Referral (if necessary)
- Copy of all MRIs, CT scans, X-rays, Vein Mapping, Vascular Studies, or any other relevant imaging
- Medical Records – including any record of previous treatments
- List of all Current Medications
If your visit is related to a AUTO ACCIDENT, you will need to fill out an extra form or two. Our staff will also need a copy of the No-Fault policy that your medical care is being covered under. If you have legal representation, please remember to bring a copy of his or her contact information.
If your visit is due to a WORKERS COMPENSATION case, there a few additional forms you will need to fill out. Most importantly, please remember to bring a copy of your Workers Compensation Board (WCB) number and Carrier Case Number with you. If you have legal representation, please remember to bring his or her contact information with you.
